Along with exceptional pay, UPH offers a comprehensive package
of benefits that is rarely matched anywhere in the health care field. Benefits are effective on the first of the month following 30 days of employment.

Benefits for half-time and greater employees include:

UPH pays a substantial portion of insurance premiums to
reduce the cost to each employee.
Single, employee +1 and family plans are available.

  • Medical Care Copay - $10 - $40 outpatient office visit.
  • Prescriptions Copay - $7 for generic drugs; $30 - $50 for brand-name prescriptions.
  • Vision - Up to $200 allowance on eyeglasses or contacts per calendar year.
  • Dental coverage through Ameritas Dental PPO Plan pays up to $2,000 per person per calendar year.
  • Flexible Spending Account - Set aside funds before taxes for health care expenses and/or dependent-care expenses through payroll deduction pre-tax.
  • Life insurance pays one times the employee's annual salary. Provided at no cost to employee.
  • Supplemental life insurance is available up to $300,000. Spousal and dependent coverage is available.
  • Long-term disability insurance provides two-thirds of pre-disability earnings after a 90-day period. Provided at no cost to employee.
  • Short-term disability insurance is available through COLONIAL Supplemental Insurance.

  • 401(k) Plan - UPH matches 50% of first 4% of an eligible employee's contribution.
    AND, UPH makes quarterly contributions of 7% of adjusted quarterly base salary.
  • Tax-deferred annuities are payroll deducted and can shelter up to 20% of earnings.

  • Tuition reimbursement is provided for eligible work-related or career development courses. There is a lifetime maximum of $15,750 and yearly maximum of $5,250.

  • PTO (Paid Time Off) accrual begins upon the first day of employment. The accrual rate ranges from 25 to 36 days per year based on years of service. It is prorated for part-time employees.
  • EIB (Extended Illness Bank) accrual begins upon the first day of employment for a total of 7 days per year for full-time employees. It is prorated for part-time employees.
  • Sick child daycare is provided in the employee's home by Choice Care Agency, Inc. The employee cost is $2.00/hr for one child plus an additional $1.00/hour for each additional sick child + mileage reimbursement.
